Important PharmaCare Grievance Information

May 14, 2014 Final Deadline for Submission of all Required Information

BCNU is reviewing PharmaCare grievances that were received by the February 27 deadline. Each grievance is being assessed to determine eligibility for remedies as outlined in the arbitration decision.

Please be advised of the following:

1. The final deadline to submit PharmaCare grievances was February 27, 2014.
2. The final deadline to submit all receipts, SA denial forms, and authorizations as well as any projected future expenses related to grievances that were filed before February 27, 2014 (i.e. if you have an ongoing prescription) is May 14, 2014.
   No extensions to this deadline will be made.
3. Any information received after May 14, 2014 will not be taken into consideration when determining possible remedies available to you.
4. If we have not received your signed authorization by May 14, 2014 we will be unable to proceed with your grievance and will close your files. By closing your file, no remedy will be available to you under the consent award.
5. Remedies for any grievances received after the February 27, 2014 deadline will be considered only after those filed by the deadline if funding under the award allows.
